Les gens utilisent la mélatonine pour ajuster l'horloge interne du corps. Il est utilisé en cas de décalage horaire, pour ajuster les cycles veille-sommeil chez les personnes dont l'horaire de travail quotidien change (trouble du travail posté) et pour aider les personnes aveugles à établir un cycle jour-nuit. La mélatonine est également utilisée pour traiter l'incapacité à s'endormir (insomnie) ; syndrome de phase de sommeil retardée (DSPS) ; trouble du comportement en sommeil (RBD) à mouvements oculaires rapides ; l'insomnie associée au trouble déficitaire de l'attention avec hyperactivité (TDAH) ; l'insomnie due à certains médicaments contre l'hypertension artérielle appelés bêtabloquants ; et des problèmes de sommeil chez les enfants atteints de troubles du développement, notamment l'autisme, la paralysie cérébrale et la déficience intellectuelle. Il est également utilisé comme somnifère après l'arrêt de l'utilisation de benzodiazépines et pour réduire les effets secondaires de l'arrêt du tabac. Certaines personnes utilisent la mélatonine pour traiter la maladie d'Alzheimer ou la perte de mémoire (démence), le trouble bipolaire, une maladie pulmonaire appelée maladie pulmonaire obstructive chronique (MPOC), l'insomnie causée par les bêtabloquants, l'endométriose, les bourdonnements d'oreilles, la dépression ou les troubles affectifs saisonniers ( TAS), déficience mentale légère, maladie hépatique non alcoolique, syndrome de fatigue chronique (SFC), fibromyalgie, syndrome des jambes sans repos, maladie inflammatoire appelée sarcoïdose, schizophrénie, migraine et autres maux de tête, perte de vision liée à l'âge, hyperplasie bénigne de la prostate (HBP), syndrome du côlon irritable (SCI), perte osseuse (ostéoporose), trouble du mouvement appelé dyskinésie tardive (TD), reflux acide, Helicobacter pylori (H. pylori), performance physique, infertilité, épilepsie, vieillissement, ménopause, syndrome métabolique, pour la récupération après une intervention chirurgicale, agitation causée par l'anesthésie, le stress, troubles du mouvement involontaire (dyskinésie tardive), modifications de la fréquence cardiaque lorsque vous passez de la position couchée à la position assise (syndrome de tachycardie posturale), délire, incapacité à contrôler la miction, douleur à la mâchoire, inflammation maladie intestinale (colite ulcéreuse) et pour le contrôle des naissances. La mélatonine nocturne quotidienne réduit la tension artérielle chez les patients de sexe masculin souffrant d'hypertension essentielle. La prise de mélatonine entraîne une réduction moyenne du cholestérol total.
